你所提供的個人資料只作與索取閉路電視監察告示範本貼紙直接有關的用途，你須為此提供有關資料。你有權要求查閱及改正公署所持有你的個人資料。如有需要，請填妥查閱資料要求表格 (OPS003)，然後以傳真 (2877 7026)、電郵 (communications@pcpd.org.hk)、親自遞交或郵寄 (香港灣仔皇后大道東248號陽光中心13樓1303室) 方式送交個人資料私隱專員公署保障資料主任收。

Personal Information Collection Statement:

The personal information provided will be used for the purpose of providing you the CCTV Surveillance notice sticker. You are required to provide relevant information to obtain the notice sticker. You have the right to request access to and correction of your personal data held by the office of Privacy Commissioner for Personal Data (PCPD). If you would like to make a data access request, please complete the Data Access Request Form (OPS003), and then send the request to Data Protection Officer of the PCPD by fax (2877 7026), by email (communications@pcpd.org.hk), in person or by post (Room 1303, 13/F, Sunlight Tower, 248 Queen's Road East, Wan Chai, Hong Kong).
